PLEASE FAMILIARIZE YOURSELF WITH THE FULL CONTENT OF THE CONSENT EXPRESSED:
„I consent to the processing of my personal data by RCP Koala (ul. Retoryka 1/17, 31-108 Kraków) in accordance with the provisions on the protection of personal data (the Act of August 29, 1997 on the protection of personal data, uniform text of June 17, 2002 Journal of Laws No. 101, item 926, as amended) in order to subscribe for a course, workshop or other type of service and when it is necessary to take action before making such a subscription, in order to settle the ordered services.
According to Art. 13 sec. 1 and sec. 2 of the general regulation on the protection of personal data of April 27, 2016, we would like to inform you that: – the administrator of your personal data is the Koala Family Obstetric Center with its registered office at ul. Retoryka 1/17, 31-108 Krakow;
– the data protection officer at RCP Koala is Ms Magdalena Owczarek (iod@koala.krakow.pl);
– Your personal data will be processed in order to sign up for a course, workshop or other type of service and when it is necessary to take action before making such a subscription, in order to settle the services ordered.
– your personal data will be stored by the Koala Family Maternity Center for a period of 24 months;
– you have the right to access your data and the right to rectify, delete, limit processing, the right to transfer data, the right to object, the right to withdraw consent at any time without affecting the lawfulness of processing (to exercise the rights and report an incident, contact the DPO at iod@koala.krakow.pl);
– you have the right to lodge a complaint with the GIODO when you believe that the processing of your personal data violates the provisions of the general regulation on the protection of personal data of April 27, 2016;
– providing your personal data is voluntary, but necessary for the registration process and participation in the birth school course, workshops and other services provided by RCP Koala;
– Your data will not be processed in an automated manner or profiled.
